Head Back to School With an Up-To-Date Shot Record

Back-to-school preparation can be somewhat daunting , especially when multiple children require a different set of supplies and uniforms. But before you stress over whether to purchase a bradded folder or three-ring binder, be certain that before your kiddos go back to school, they are up to date on their immunizations. Signed proof of current immunizations is by far the most important supply on your child’s back-to-school list.

Required Vaccines Grades Kindergarten-12th
3 to 5 doses of DTaP/DP/DT/Td (diptheria, tetanus, perussis)
3 to 4 doses of polio
2 doses of MMR (measles, mumps, rubella)
3 doses hepatitis B
1 to 2 doses varicella
1 Tdap (booster)

Additional Vaccines for Kindergarten and 1st Grade
2 doses hepatitis A
2 doses varicella

Additional Vaccines for 7th and 8th Grade
2 doses varicella
1 Tdap (booster)
1 dose meningococcal

If You Have Chosen Not to Immunize...
If you have elected not to vaccinate your child(ren) for personal reasons, you will have to request and complete a vaccine exemption affidavit listing students’ name, date of birth, relationship to child, and notarized signature stating you are aware of the benefits and risks of vaccination. Free and Discounted Immunizations
Troubleshooters Program: This program provided by the Harris County Hospital District offers free immunizations for children age 2 months to 1 years during the months of June, July and August from 9 a.m. to 1 p.m. at various locations throughout the Greater Houston area. Sites include community centers, YMCA branches and schools.

Blue Cross Blue Shield Care Van: This free immunization service is provided to children age birth to 18 years. Find the van at several Houston area sites each month. To find the Care Van nearest you, call1.800.258.5437.
